Responsibilities:

  • Full time case manager needed to assist homeless individuals living with HIV/AIDS, mental illness, and/or substance abuse to seek and maintain permanent housing.

  • Responsible for coordinating supportive services in the community for housing, primary care, mental health and substance abuse recovery needs.

  • Assist clients with development and monitoring of individual service plans and access to mainstream resources to foster self sufficiency.

  • Demonstrates knowledge of client issues related to specific service issues and needs.

  • Expresses working understanding of community based case management and its role in the program's multi-disciplinary treatment team.

  • Demonstrates well-developed communication skills at a variety of levels. Interacts effectively with consumers, peers and other professionals.

  • Maintains confidential relationships and professional boundaries with consumers and peers at all times.

Qualifications

  • Grade 5: Case manager I -HSD/GED or HiSetrequired; associate's degree preferred.

  • Grade 6: Case manager II-Bachelor's Degree required; degree and/or course work in social work, counseling, psychology or related field preferred

  • Experience with homeless clients, HIV/AIDS, Alcohol and/or Substance Abuse and/or Major Mental Illness is desirable.

  • Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle

  • Prior case management experience preferred.

  • Bilingual English/Spanish skills helpful.

  • Must be able to Pass a CORI background check
