  • Eligible candidates are hired (offer accepted) into a Full or Part time Case Manager position. Bonus amount prorated for Part time hires, per diem hires are ineligible.

Maine state certification as a MHRT/C is required (unless clinically licensed), Maine state licensure to practice as a LSW preferred

This is an exciting Full-time, 40 hours per week, benefit eligible opportunity to be a part of a dynamic, recovery focused, multidisciplinary team serving clients with co-occurring serious mental illness and substance use disorders.

Assertive Community Treatment (ACT) is an intensive community based (outpatient) treatment program with a multi-disciplinary team of providers that supports eligible adults and children who may have a major mental illness diagnosis and often co-occurring disorders. The goal of the ACT Program is to improve the client’s functioning in the least restrictive setting, while strengthening family, work, school and community ties.

Position Summary

The Assertive Community Treatment (ACT) team is dedicated to providing mental health rehabilitation services, supporting adults and families to reduce hospitalization and crisis episodes.  The ACT Clinical Case Manager, operating under the guidance of the ACT Team Program Manager, delivers essential services, primarily in the clients living situation, including crisis intervention, case management, individual coaching, monitoring for medication adherence, and skills training. In the community and on call situations, the case manager exercises the experience and judgment required to make sound clinical decisions independently.  Committed to ethical, culturally competent diverse and inclusive practices, the case manager ensures client service delivery aligns with regulatory, accreditation and best practice standards for optimal patient centered customer service satisfaction.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

    1. Education: Bachelor’s degree from an accredited institution with a specialization in psychology, behavioral health, social work, special education, counseling, corrections, rehabilitation, nursing or closely related field is required.  2+ years of experience in the same areas of specialization; or 5 years of experience working with people diagnosed with severe mental illness.   
    2. License/Certifications: MHRT-C certification required unless clinically licensed; Eligibility for LSW licensure preferred; Valid Driver’s license and appropriate level of automobile liability insurance.
    3. Experience: 5+ years of experience working with a broad spectrum of mentally ill clients is preferred. See also "Education"
    4. Excellent computer skills are required: Microsoft Word, Excel and use of electronic consumer record software
    5. Knowledge and commitment to abide by the Code of Ethics of the National Association of Social Workers
